Hearth, Inc. is a non-profit organization dedicated to the eradication of homelessness among the elderly of Greater Boston and beyond. Hearth is unique in its development and operation of permanent housing services specifically designed to support formerly homeless elder’s age with dignity. Hearth also provides outreach, placement, case management, and advocacy for elders who are homeless or at high risk of homelessness.



Full time position for a registered nurse to provide care coordination of all health services for residents of a assisted living site. Work as part of a multidisciplinary care team to support frail elders in the community. Responsibilities include health assessment and screening of individual clients, managing the delivery of personal care homemaking services by CNAs, medication management oversight, coordination of resident health care services. Clinically grounded RN, BSN preferred, with two years experience including some acute care experience. Community and mental health experience preferred. Experience with GAFC, DMH and/or MAP requirements helpful.
