Riverside Community Care is a long established and growing provider of innovative mental health care, mental retardation and head injury services, substance abuse treatment and other human services to adults, children and families in a wide variety of outpatient and residential settings across 60 communities in Eastern and Central Massachusetts.





We are seeking staff to provide outreach to consumers via the phone and face to face meetings, in their apartments and community settings. Maintain regularly scheduled appointments with consumers. Works as a part of a supported housing program in providing guidance, education supervision and direct support to individuals with mental illness. Advocate for and support consumers in identifying and accessing the full range of community-based supports. Provide intervention and support strategies to assist consumers with crisis prevention, symptom management and the development of self-managed coping skills.





Requires a bachelor's degree plus six months of related experience, or at least three years of related experience. Car and valid driver's license is required.
